Registration Information

Our focus is to make the registration process pleasant and convenient for our patients. We offer a wide variety of outpatient services, including walk in lab. Our hospital also provides high quality inpatient care. The enhancement of the Central Scheduling department gives patients and physicians the opportunity to schedule Radiology tests at the convenience of the patient.

Upon arrival for outpatient services or inpatient stay, you will be greeted by a registration representative who will verify your current information. You will be requested to sign a "Consent for Treatment" and "Consent to Bill" form. You may also be asked to sign other forms that may be required by Medicare or other regulatory agencies. All patients will receive the "Patients Bill of Rights".

Please have the following items ready when you arrive, as this will insure that the registration process flows smoothly and efficiently:

  • All current health insurance information
  • Picture ID
  • Physician orders or prescriptions for any test
  • Referrals from your PCP that may be required by your insurance carrier
  • Living Will (if you are scheduled for surgery or are arriving for inpatient services )
  • Pre-admissions Interview (for scheduled surgery)
  • Personal Medication Record Form
